On March 14, 2017, BioReference Laboratories, Inc. (BRLI) discovered that one of its employees discarded documents in a dumpster in Davenport, Florida that contained patient protected health information. Although this employee was required to shred and securely dispose of these documents, they were left in a dumpster in violation of our company protocol. The records involved in this incident contained patient first and last names, dates of birth, addresses, medical record numbers, social security numbers, insurance information, diagnosis codes and descriptions of the lab tests ordered. These records did not contain any financial information and we have found no evidence that any patient personal information has been improperly used or accessed.
Since learning of this incident, BRLI immediately recovered the records containing PHI from the dumpster, undertook an internal investigation and terminated the responsible employee. Our investigation into this incident will continue and we will make all necessary changes to our safeguards and policies, and retrain our staff. Affected individuals should remain vigilant to prevent misuse of their personal information, consider monitoring their financial statements and reporting any suspicious activity, and consider using the credit monitoring services that BRLI is offering to all potentially affected individuals at no cost.
